The ULA MPLA Joint Conference will be held on April 29th - May 2nd.
You are invited to attend and USL will pay for your ULA membership, registration fees, and meals associated with the meeting. Please note that state travel rules apply for mileage and per diem so please consult MapQuest to determine eligibility for housing, mileage and per diem. 1. Complete the ULA membership form for new ULA membership or to renew your ULA membership. A PDF of the form is attached. Send or fax your completed form to the State Library.
2. Go to the following site and PRINT the page:

DO NOT COMPLETE THIS FORM ONLINE. INSTEAD PRINT THE FORM AND THEN FILL OUT THE PRINTED FORM. SEND OR FAX YOUR COMPLETED FORM TO LISA ROGERS AT THE STATE LIBRARY.
Lisa will arrange for your ULA membership and registration for the conference and payment for both.
